Lola’s New Beginnings provides services to all Medicaid eligible pregnant women and women with newborns . Women who are interested in learning more about having a healthy baby are eligible to participate in PNCC/CCC program.
Our program serves residence throughout Milwaukee County.

Care Coordinators work with families to identify their goals, support families in efforts to meet their goals, provide information on how to care for self and baby and connect families with community services in their neighborhood.

Care Coordinators will accomplish these tasks by offering the services listed below:

• Health assessments and information.
• Goal setting and care planning
• Advocacy
• Provide referrals to community resources.
• Complete home visits
• Take referrals
• Provide health and nutrition classes
• Provide parenting classes
• Assist with concerns such as employment, education, W2, and WIC
• Assist mothers with locating free or low-cost baby items such as diapers, wipes, clothing, cribs, formula, and car seats
• We continue with coordination of services until the child reaches 7 years old
• Housing Updates & Opportunities.

If you are detailed focused, a self-starter, are good with time management and have great organizational skills we are looking for you.
