Lola's CCC program is here to improve birth outcomes among women who are deemed at high risk for poor birth outcomes.

Lola's Child Care
Coordination

Child Care Coordination services help mom and her family gain access to medical, social, educational, and other services related to the recipients needs.

Wisconsin Medicaid CCC services are available to Medicaid eligible women after pregnancy until the qualifying child is seven years old.

Prenatal Care Coordinator Goal

Our goal is to help guide you to achieve yours

Each mom will:

Receive individual psychosocial support and services.

Receive continuous Child Coordination Care services.

Receive necessary health and nutrition education. Are referred to available community services, as appropriate.

Receive assistance in accessing and obtaining needed health and social service.

Receive access to the many resources Lola's has to help you reach your goals.

What can I expect?

Child Care Coordination Services include all of the following:

Initial assessment.

Care plan development.

(Where do we want to be?)

Ongoing care coordination

(Motivation and progress check in)

Health education and nutrition counseling services

(Identify needs).

Materials needed to overcome challenges.
